End User License Agreement

An end user license agreement (EULA) form will appear after requesting an account. There are three possible actions:

• Accept – forwards the user access request to the healthcare provider; you will not be prompted again with the EULA form on future logins
• Defer – forwards the user access request; you will continue to see the EULA prompts on future logins, until you accept
• Decline – the access request is not submitted

If you select Accept, then a Registration Complete message will display. Once your healthcare provider grants access, you will receive a confirmation email, with a security code that you enter on the first portal login.
